Request an appointment

To ensure you receive the right care as efficiently as possible, our online triage system, remains in place for all appointment and consultation requests.

Please submit your requests via our online triage system  instead of calling the practice.

How It Works

  • Contact us online
  • Choose the type of request – Medical or Admin.
  • Complete the form with as much detail as possible.
  • Submit your request, ensuring your contact details are up to date.

Your request will be reviewed by a GP, who will determine the best next steps. This may include:

  • Booking an appointment with a GP or another clinician.
  • Providing advice or treatment remotely, where appropriate.
  • Directing you to a more suitable service, such as a pharmacy or physiotherapist.

What You Can Use Patient Triage For

  • Appointment requests
  • Test results
  • Prescription queries
  • Sick notes & medical letters
  • General medical advice
