How to Make and Cancel an Appointment

Answer a few short questions and we will get back to you within 3 working days. Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during office hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday-Friday between the hours of 1800 - 0900. Please contact 111 during out of hours or 999 if this is an emergency.


Emergency appointments

Emergency appointments are available on the same day, please aim to ring at 8 a.m. As these appointments are limited they are allocated on a priority basis. If you think your medical problem is an emergency e.g. severe chest pain, breathing difficulty then please tell the GP/ Receptionist so they can decide on the most appropriate course of action.



Extended hours

If you are unable to attend an appointment during normal surgery hours Monday to Friday 8.00am to 18.30 due to work commitments there is a surgery available at each site on alternative Saturday mornings from 8.00am to 1.00pm. Please ask the receptionist for further details.


Telephone appointments

A Doctor is available for telephone consultation during surgery opening hours. This may benefit those who may not necessary need to see a doctor but require further medical advice. For example, information regarding medication, discussion of results, medical certificates.
