We have an open list and welcome new registrations from patients living in or moving to Addiscombe, East Croydon and West Croydon areas.
You can register with us by completing this GMS1 form and Questionnaire and emailing them to SWLCCG.stjamesmedicalcentre@nhs.net / SWLCCG.castlehillsurgery@nhs.net
Photo Id and proof of address also needed and return in person in order to process the registration.
All patients on routine medication should make an appointment with a doctor well before their medicaion runs out to ensure continuity as prescriptions will not be issued without seeing a doctor.
Live within boundary area?
To be eligible to register at the practice, you must live within the practice catchment area.
Named Accountable GP
From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a named accountable GP.
St James Medical Centre has always allocated a named usual GP to patients and this Doctor will be your named accountable GP.
If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery.
Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.
